REPORTING

REPORTING:
​Disqualifying a Player or Coach:

​When writing a Misconduct Report or an Incident Report, include Referee #2 and/or Assistant Referee's/Witness/s, and in the Detailed Explanation include the actual game time (ie, 35th minute, 55th minute, 5th minute second half, etc.).

ALL MISCONDUCT AND INCIDENT REPORTS must be filed by the evening of the game.  NO EXCEPTIONS.  The reason being is that I need to review them and forward them to Anita Diaz by 8:00 am the following day.

• Print copy before the next step
• Select Nico Esquivel as our Unit's Liaison from the Liaison drop down list.
• Players receiving double yellow cards do not need to be reported
• Coaches receiving double yellow cards must be reported as a straight red card on the form.
• Print form first before you save, as this will allow you to print  and send e-mail the form.
• Once you hit save, the form is sent to CIF
• E-mail a copy of the report to Nico Esquivel,  Ian Anderson and Mike Skinner

 
Incident Report for any Serious Injury:

• An Incident Report must be filed by the evening of the match for any serious injury or possible concussion.  NO EXCEPTIONS.
• Incident Report
• Select Sport (soccer)
• Password: 123
• Print form first before you save, as this will allow you to print and save or e-mail the form.
• Once you hit save, the form is sent to CIF
• E-mail a copy of the report to Ian Anderson, Mike Skinner and Nico Esquivel
